* Required. Your colleague needs an email or mobile number, or both.
Program terms.
- Who can refer: any physician, CRNA or advanced practice provider who works with Falcon Locums or has applied to. Falcon Locums employees and owners are not eligible.
- Qualifying referral: the referred provider must be new to Falcon Locums (not already in our database and not contacted by our team in the past 6 months) and referred before they apply or are contacted by us. If two people refer the same provider, the first referral we receive counts. One bonus is paid per referred provider.
- Bonus: $2,000 for an emergency medicine or anesthesia physician, $1,500 for other physician specialties, $1,000 for a CRNA or advanced practice provider. Earned when the referred provider completes their first 10 shifts with Falcon Locums within 12 months of the referral.
- Payment: within 30 days of the milestone, by check or direct deposit. A completed W-9 is required; bonuses are reported on Form 1099. If you choose a charity, we donate the bonus amount to a registered 501(c)(3) of your choice.
- Luggage tags: the referrer receives a personalized luggage tag once the referred provider has a first call with our team. The referred provider receives a matching tag when they start their first assignment.
- Program limits: bonuses are paid only for referring providers to work with Falcon Locums. No payment is made for referring patients, or for introductions to hospitals, health systems or other facilities. The bonus never depends on where a referred provider works. You are not eligible for a referral placed at a facility where you work, contract or influence hiring. Referral bonuses create no ownership, equity or partnership interest in Falcon Locums.
- Permission and disclosure: you must have your colleague's permission before referring them. If you promote this program publicly, you must disclose that you may receive a referral bonus.
- Changes: Falcon Locums may update or end this program at any time. Referrals made before a change are honored under the terms in place when they were made.
